Abstract:

Introduction
The identification of patients who need cholesterol-lowering treatment is actually based on complicated algorithms. The aim of the present study is to provide an easy tool to identify patients at high cardiovascular risk (CVR).

Methods
The results are based on 946 primary care patients aged between 45 and 65 years and without a history of cardiovascular disease. The CVR estimated with several risk scales based on the usual dichotomized risk factors was compared with the CVR estimated with the Framingham and European and Belgian SCORE tables.

Results
The Five Risk Score (5RS) takes male gender, systolic blood pressure, total cholesterol, and smoking into account as independent risk factors. Diabetes is considered as an exclusion criterion for which treatment with lipid-lowering drugs is always indicated. Patients with two independent risk factors or diabetes qualify for lipid-lowering treatment. Compared to the Framingham risk algorithm, the 5RS has a sensitivity of 100% and a specificity of 82%. Compared to the Belgian SCORE tables the 5RS has a sensitivity of 93% and a specificity of 85%.
For the European SCORE the best results were obtained with the Six Risk Score (6RS). This 6RS uses the same independent risk factors but diabetes as well as a family history are considered as exclusion criteria. Compared to the European SCORE, the 6RS has a sensitivity of 100% and a specificity of 92%.

Conclusion
The 5RS and the 6RS are useful alternatives for the existing algorithms. They easily identify persons for which lipid-lowering treatment is necessary.
