Abstract:

INTRODUCTION
Hypertension is the most important but modifiable risk factor for stroke. Despite the overwhelming evidence that lowering blood pressure (BP) reduces the risk of cardiovascular (CV), cerebrovascular and renal morbidity and mortality high BP remains treated suboptimally. Hypertension is often associated with other CV risk factors, therefore reducing patients CV risk needs the management of all these risk factors.

To evaluate the impact of an online risk calculator (« tool ») for the 10-year estimated Framingham stroke risk and the 10-year estimated CV mortality risk (SCORE) on the risk profile of hypertensive patients not at goal for their BP up to one year after its implementation. This poster reports the baseline characteristics of the study.

Prospective study in primary care from May 2005 to February 2008. Men and women > 40 years, with BP not at goal, defined as systolic BP (SBP) >140 mmHg and diastolic BP (DBP) >90 mmHg for all patients and SBP >130 mmHg and DBP >80 mmHg for the diabetic patient. Duration of the study was 28 months, patients were seen every 4 months.
The 10-year estimated Framingham stroke risk was calculated for patients aged 55-84 and free of previous stroke using age, gender, SBP (treated or not treated), smoking status, CV disease (past and present), diabetes mellitus, atrial fibrillation and left ventricle hypertrophy. Framingham stroke risk score: low < 10%, moderate 10-19%, high ≥ 20%.
The 10-year estimated CV mortality risk (SCORE) was calculated for patients aged 40-74 and free of an atherosclerotic event or diabetes using age, gender, SBP, smoking status and total cholesterol.
SCORE risk score: low < 2%, moderate: 2-4%, high: 5-9%: very high ≥ 10%. The risk of each patient was calculated using a specially designed interactive website.
