AIM: To examine total cholesterol (TC) and LDL-C goal attainment at baseline and after treatment of the CHD patient, together with the percentage of these patients with elevated risk factors such as low HDL-cholesterol (HDL-C) and elevated triglycerides (TG).
METHODS: This analysis was a secondary endpoint of the Belgica-Stroke study, a prospective, multicentered, primary care study (2005-2008) in Belgium including hypertensive patients aged 40 years or older, not at goal for their blood pressure. The goal attainment was calculated following the ESC guidelines 2003-2007 and ESC-EASD 2007.
RESULTS: 15980 patients were recruited, of whom 15744 met the inclusion criteria; 2939 had CHD, with 1071 (36%) concomitant diabetes mellitus (DM).
The 175 mg/dL target for TC was reached in 25% of patients with CHD without diabetes and in 35% of CHD patients with diabetes. The 100 mg/dL target for LDL-C was reached in 32% of patients with CHD without diabetes and in 43% of CHD patients with diabetes. Treatment, in 85% statin monotherapy, increased goal attainment for LDL-C versus no treatment. Risk factors such as high TG's, and low HDL-C were elevated, especially in the diabetic group.
CONCLUSIONS:. LDL-C goal attainment was poor among the CHD patients, with at least half of them not being at goal. Treatment increased goal attainment for LDL-C versus no treatment. More than half of treated patients did not achieve the TC and LDL-C goal, despite statin monotherapy in 85% of the treated patients.
